For services, choose “UDUTH School of Midwifery.” Confirm that the service amount is N5,000, and complete the required details before selecting “Generate Payment Instruction.”



3. After generating the payment instructions, proceed to any bank for the actual payment. The bank will give you a teller after successful payment.

4. You will then go ahead and give the teller and the Remita Slip to the SOM-UDUTH cashier to obtain a receipt.

5. The receipt, along with a copy of the Remita Slip, should be taken to the SCHOOL OF MIDWIFERY UDUTH SOKOTO. That is where you will receive your unique Serial Number and Pin.

6. You can now access the application portal now you have these details. Enter your details accurately and submit your application.

7. Remember to keep a printed copy of this filled form. You will need it for the Entrance Exams. It will serve as your ticket to the first step in your exciting journey toward becoming a skilled midwife.
